Prevention Theme Overview
|
The overall goal of the Prevention Theme is to improve the delivery of preventive healthcare services in order to optimize healthcare efficiencies and improve the quality of life for Medicare beneficiaries. The focus on prevention allows providers and QIOs like SDFMC to build on progress they have made over the past three years. Under this theme, we will work with providers to improve the rates of colorectal cancer and breast cancer screenings, as well as immunization for influenza and pneumonia among Medicare beneficiaries in South Dakota.
SDFMC activities under Prevention Theme will focus on these primary topics:
Administering an assessment of care processes survey.
Providing on-site education to improve care management for practices using ambulatory electronic health records that are certified by the Certification Commission for Healthcare Information Technology (CCHIT)
Assisting with data submission to the CMS-designated repository
Monitoring statewide rates for colorectal and breast cancer screenings, as well as influenza and pneu-mococcal immunizations.

Requirements for Participation
|
Have a CCHIT-certified E.H.R in place as of 10-31-08
Provide information to SDFMC about care processes.
Report quality measures at least quarterlybreast cancer, colorectal cancer screening and influenza and pneumococcal immunization data.
Complete a minimum of two-hours of education on health care quality.
