Physician Office Project
CMS is committed to working with providers, in the physician office setting to improve the quality of care for Medicare beneficiaries. South Dakota Foundation for Medical Care (SDFMC) will work with physician practices and stakeholder groups to improve care for Medicare beneficiaries, on a statewide basis, in the areas of chronic care for disease (diabetes) and preventative services (cancer screening and adult immunization).

Care for Chronic Disease (Diabetes) Care Measures:

  • Annual HbA1c testing

  • Biennial testing of lipid profile

  • Biennial retinal exam by an eye professional


  • Preventative Services Care Measures:

  • Biennial mammography screening

  • Influenza immunization

  • Pneumococcal immunization


  • SDFMC will also work more intensely with an identified group of physician practices to promote system changes, including adoption and effective use of information technology (IT) such as electronic health records (EHRs), electronic prescribing and reminder systems, in conjunction with redesign of practice processes. This effort will focus on reliable delivery of preventative services and effective management of patients with chronic conditions (diabetes and heart disease). In this effort SDFMC will provide technical assistance to include:

  • Adoption and effective use of health information technology (HIT)

  • Process redesign which includes care management of patients with chronic conditions and preventative services

  • Measurement and reporting of specified clinical quality measures and improvement on those measures
