Case management is a collaborative process of assessment, planning, facilitation, and advocacy for options and services to meet an individual’s health needs through communication and available resources to promote quality cost-effective outcomes.

Objectives
1. Identify strategies of physician directed case management and model and integrate successful physician directed case management strategies in a home health agency.
2. Identify the role of nurse case managers in a home health setting.
3. Identify the benefits and barriers of a Case Management Model in a home health setting.
