Objectives
1. Review the success of reducing avoidable hospitalizations of home health patients by identifying home health patients at high risk for hospitalization upon admission to home health.
2. Examine successful patient emergency care planning that resulted in reducing avoidable hospitalization of home health patients.
3. Discuss how one agency reduced hospitalization of home health patients through a program of frequent phone follow-up monitoring.
