A South Dakota home health agency has shared a wallet-sized medication card which they use across the continuum of care throughout their healthcare facilities. The agency utilizes bright yellow card stock to print these cards. The electronic version contains two cards on an 8.5” by 11” sheet. The medication card is meant to be printed double sided and folded to wallet size. Medication wallet card The agency has also provided a cover letter targeted to healthcare providers.
